Turkish medical report: public hospital or private clinic

A Turkish medical report’s apostille route depends on the issuer: a public hospital’s report is apostilled directly, a private clinic’s via a notary.

A medical report from Türkiye is asked for abroad in files for a work permit, residence, adoption, school enrolment, insurance and a driving licence. Who issued the report settles the apostille route from the start: a report from a public health institution is an administrative document and is apostilled directly, while a report from a private hospital or practice is not a public document in the sense of the Convention and enters the chain through an official certificate placed on it, such as a notary’s certification.

A report from a state or university hospital

A report drawn up by a state hospital, a university hospital or the il sağlık müdürlüğü, the provincial health directorate, is an administrative document when it bears the signature of the responsible physician and the seal of the institution. The Zile District Governor’s Office writes that documents drawn up by departments attached to the governor’s office, the provincial health directorate among them, are certified at the governor’s office when they are to be taken abroad. The Onikişubat District Governor’s Office writes that it certifies documents issued anywhere in Türkiye, while the Develi District Governor’s Office lists only documents from its own district; the “any province or district” rule of article 199 of the Notaries Law covers notarial acts, and a uniform practice for administrative documents was not verified in this site’s survey, so the office is asked first. Where several physicians have signed, the apostille attests the signature it names; on a medical board report the chief physician’s endorsement matters for that reason.

A report from a private hospital or practice

A report from a private hospital or a private practice is signed by a physician in a private capacity. The Convention treats official certifications placed on documents signed by private persons as public documents: the report is either signed before a notary or copied by the notary as a certified true copy (aslı gibidir), and the apostille is placed on that notarial act. The apostille then attests the notary’s signature and capacity, not the physician’s, and the medical content is never verified at any stage. If the receiving institution insists on a report from a public institution, the notary route does not satisfy that condition.

Electronic reports and the barcode problem

A medical report may be issued electronically and downloaded from e-Devlet, the Turkish e-government portal, as a barcode printout. A plain barcode printout receives no paper apostille. The rule of the Onikişubat District Governor’s Office applies here too: if the issuing institution physically certifies the e-signed printout with the name, title, handwritten signature and seal of its official, the document can be considered, and the apostille then attests that official’s signature. A medical report is not among the document types covered by e-Apostil, the official Turkish electronic apostille system.

The provincial health directorate pre-approval question

Some sources state that a medical report has to be approved by the provincial health directorate before the apostille. This site’s survey found no official regulation or official page imposing that condition; what is published is that documents issued by the provincial health directorates themselves are certified at the governor’s office, which is not an intermediate approval. Since practice can change, the governor’s or district governor’s office that will apostille the report is asked before the visit; this site does not present an unverified condition as a rule.

Medical terminology and the report’s short life

The report is translated by a sworn translator (yeminli tercüman, whose oath is recorded at a Turkish notary) and the translation is certified by the notary; medical terms are rendered in the classification used in the receiving country. If the institution also wants an apostille on the translation, that second apostille is placed on the notarial certification. Where the report carries its own period of validity, the apostille does not lengthen it, so the report, the apostille and the translation are planned together against the application date. In the French Foreign Ministry’s legalisation table administrative documents from Türkiye need an apostille; a medical report is not within the civil status exemption.

Translation in the chain

Where does the translation fit?

  1. Document going abroad The sources describe two routes: an apostille on the original, or a translation certified by a notary with the apostille then confirming the notary’s signature. The receiving authority says which it wants.
  2. Foreign document used in Türkiye The sources describe an apostille in its country first, then a Turkish translation with notary certification; the Turkish institution receiving it confirms its own requirement.
  3. Sometimes none is needed Multilingual civil status extracts and some treaties remove the translation or the apostille. The authority that receives the document decides.
